NJWELL is offering various wellness incentive levels to active SHBP and SEHBP members and their covered spouses or partners who participate in the state’s Direct Primary Care Medical Home program.

MEMBERS AND COVERED SPOUSES OR PARTNERS CAN EACH EARN $250 – $500.

Any active member of R-Health by Everside Health will automatically qualify for the NJWELL reward card when you:

  • Complete a health assessment through your health plan’s online portal
  • Complete a comprehensive physical examination and at least one follow up appointment with your R-Health by Everside doctor or complete two virtual visits (with one focused on prevention and wellness)
  • Create a health map in partnership with your R-Health by Everside doctor
  • Complete all of the above between November 1 and October 31

How much can you earn with NJWELL and R-Health by Everside?

  • $250 for active SHBP and SEHBP members
  • $350 for active SHBP members with the CWA Unity, NJDIRECT, NJDIRECT2019, OMNIA
  • $500 for active SEHBP members with NJDIRECT Zero plans
