The Advanced Comprehensive Care Organization (ACCO) is R-Health’s first Accountable Care Organization. And it’s a remarkable one. This network of family, pediatric and internal medicine practices is out to reform healthcare three ways: better care, better health, and lower cost for patients with chronic conditions.

Using the Patient Centered Medical Home (PCMH) model and the Chronic Care Model, these practices combine to

  • Empower patients to engage in their own care
  • Redesign office workflow and roles
  • Implement evidence-based guidelines to reach patient goals
  • Use technology to monitor quality and manage populations
  • Reduce avoidable hospitalizations, re-admissions and ER care
  • Continuously improve quality
  • Create performance based incentives and new payment models
